Cancer Survivor Care Plans
Cancer survivors and their care providers often lack sufficient information about how to address their short and long term medical needs. A number of care plan models are available. How can we work to ensure that cancer survivors have a care plan?
Cancer Plan Minnesota Objective 17: Optimize the continuity of care for cancer survivors during and beyond the initial course of treatment.
Strategy: Develop and promote methods to facilitate the exchange of information among all healthcare providers involved in the care of cancer survivors.
Primary Aim: To engage and enlist additional people and resources in Cancer Alliance activities. In specific, to address need for survivor care plans.
Desired Outcomes:
· Identification of elements of a survivor care plan
· Come to consensus on a preferred survivor care plan
· Commitment of individuals to do something
