This project uses the Metropolitan Health Plan (MHP) member data as a means of identifying people who are in need of CRC screening.  The intervention is done for lower income plan members (similar to UCare Minnesota’s Medicaid group) and includes a $40 Target gift card as an incentive.  The project has been in operation for two years and will continue for one more year.  The funding is provided by DHS.  The response rate so far is 11%.
