LAF grant proposal – Jody Jackson
Our LOI was well received and we’ve been asked to submit a full proposal
3 main objectives
Determine essential elements for MN SCP
Create short term care plan – address medical, emotional & practical needs
Create long term care plan – address medical, emotional & practical needs
Next steps
Create the team to write the grant
Could focus groups be pulled from our various organizations?
Lindy Yokanovich & Cancer Legal Line also applying for LAF grant
American Cancer Society’s Survivorship Advisory Group – Freda Carlson
ACS Mid-Atlantic region is also working on a SCP
Karen Babb in Virginia is heading up the effort
They’ve created a plan, obtained Dr feedback, working on patient feedback, will do focus groups
We hoped they’d share their draft but they will not at this time
Cancer Summit 2008 Survivorship breakout session was highly successful
Our SCP should be portable, not unwieldy, customizable,
Divided into 2 parts: 1)Basic info & 2)Additional info
–or- 1)Short term & 2)Long term
We could include a checklist of things to think about with ability to give additional info

Look at Interagency Transfer Form model
Included in every chart from beginning because they know the patient will eventually be discharged
Introduce SCP early in process: at initial oncology visit
Patient Responsibility, Ownership, Empowerment
Goal: Facilitate Accurate Communication, normalizes experience

Paula Colwell shared the binder & tool she created for use at Virginia Piper
Everyone is different with different needs so we’ll need different plans – we can’t standardize
SCP will:
not be a duplicate of medical records
be patient & family focused
change over time
How can our SCP meet the needs of Survivors, Oncologists, Primary Care Physicians
Logistical Questions:
When to introduce?
Who fills out what parts?
When is it handed off?
What size/how big?
Challenges of 1 size fits all: disease site, care setting, urban vs. rural
Anna will work to put the elements included in the SCP draft & from the Summit into the grid for our next meeting
