  • Cancer Plan Minnesota’s Quality of Life Chapter was reviewed
  • Possible project ideas were brainstormed
  • An online survey will be created to determine which project ideas have enough interest for groups to be formed to carry them out

Project Idea Brainstorm & Discussion

·   Advocate at MN Legislature for Medicare or UCare to pay doctors for their time completing a document that outlines a comprehensive cancer survivorship plan including descriptions of medications and side effects, follow up tests needed, guidelines, etc.

·   Create a Survivor Care Plan showing ROI

o        Primary Care Physicians feel there is a need for improved communication

o        A partnership with the ONS & AAFP could be beneficial

o        Greater MN health systems feel there is a need for improved communication

·   Spread information about palliative care programs in MN

·   Hold a survivorship conference

·   Promote a device like Vital Key, a flash drive containing all patient medical records

·   Highlight the importance of physical activity

·   Remove transportation to treatment barriers especially for rural Minnesotans

·   Create a survivorship clinic

·   Look into telemedicine

·   Create a Community Health Worker training to provide a link to resources for un- and under-insured people

o        Create curriculums for doctors and/or CHWs around palliative care

·   Look at Cardiac Rehabilitation model for Physical Activity / Reconditioning for survivors of cancer

·   Anna will email a link to a survey to all people interested listing project ideas to determine what project(s) the group(s) will pursue
